We present the Agent Incident Registry (AIR), a source-linked catalog containing \N{} records of agent-related events disclosed from \Yfirst{} through \Ylast{}. Each record includes supporting evidence, a stable identifier, and missingness-aware labels for causal role, disclosure class, mechanism, and outcome.
